This form is to collect information for boarding pets only. Please pre-book via PetDesk or by call the clinic: (225) 618-2202. Please fill out the information below to the best of your ability.

Your pet must be current and up-to-date on all vaccines: DAP, Lepto, Bordetella, & Rabies vaccines within the past year. We must have your records on file prior to boarding if you are not a current patient with FRVC.

Will you be bringing your own food for your pet? (we feed Science Diet Sensitive Skin/Stomach otherwise) *

If you are bringing your own food- please list: 1.) Brand 2.) Amount to Feed DailyTotal (in cup quantity: 1/2 cup, 1 cup, 2 cups, etc.) 3.) How many times a day to feed- once or twice daily?

Will you be bringing any other items with your pet? Collar, Leash, bedding, toys, treats? If so, please list below w/ description of bedding or toys.

Is your pet currently on any medication? *

If your pet is on medication, please list the name(s), how many times per day given, quantity to give (1/2 tablet or 2mls), and if it was given prior to drop off. Eye medications please list if one eye or both eyes.

Does your pet require any medical attention while here? *

If so, please explain what your pet needs: Medication(s), Doctor exam, vaccine(s). ***Grooming must be scheduled via PetDesk or by calling as these appointments are limited.***

If we can't get in touch with you while your pet is boarding, please list an emergency contact: Name & Phone Number with area code (225-XYZ-1234).

If there is an emergency with your pet and we cannot get in touch with you or your emergency contact (please ensure your ER contact is approved to consent to costs for emergency treatment), would you like us to perform life saving measures up to $250 without your consent? Any charges after that will need your verbal consent unless otherwise stated. *
